Abdomen – Dermolipectomy

A Dermolipectomy is a surgery that removes excess fat and skin from the abdomen region. The name can be frightening, but the concept is simple: the prefix "dermo", from Greek derma, means skin; "lip" comes from lipo, and means fat. The term "ectomy", in turn, comes from the Greek ek take, which means removing through surgical incision. In practice, the procedure is performed with an incision, depending on the volume of the stomach, which goes from one iliac crest (hip bone) to the other.

Also known as abdominoplasty, this technique is suitable for people who have a very large abdomen, or who have lost a lot of weight and have "leftover skin." In such cases, it is not enough just to reduce the volume of the belly fat, it is also necessary that the skin is stretched over and cut. With some who is younger and has never had children, the abdominal muscles are still attached to each other. Over the years, factors such as an inactive lifestyle and pregnancy cause the weakening of these muscles, leaving a flabby belly that projects forward. To resolve this problem, we apply a series of stitches that rejoins the muscles, leaving them with the original form, reducing the sagging of the muscles.

HOW MANY POUNDS WILL BE LOST? Being a surgery which removes a certain amount of skin and fat of course there will be a reduction in body weight, which varies with the volume of the abdomen of the patient. These aren’t however, the "pounds" that define the aesthetic result, but to maintain the proportions of the abdomen with the rest of the body. Most women experience some loosening of the abdomen after one or more births, with a lot of skin and fat located in the region. These cases yield better results. If the patient is overweight, the result will be compensatory and proportional to the rest of the body, but it is worth remembering that "excess fat" in other neighboring regions of the abdomen will still exist, which leads us to advise them that they have to continue with further treatment, either by liposuction or even clinical or physical therapy, to balance with other areas. The diastase of the abdominal muscles (that bulge in the central region of the abdomen) is treated simultaneously with this abdominal surgery.

AND WHAT ABOUT THE SCARS? The scar of a Dermolipectomy is located horizontally just above the area of pubic hair, extending laterally to a greater or lesser extent depending on the volume of the abdomen to be corrected. This scar is planned to be disguised beneath the underwear, and passes through several stages as follows: IMMEDIATE PERIOD: Up to the 30th day with little visible aspects. Some cases have a mild reaction to the stitches or bandages.
MIDDLE PERIOD: From the 30th day until the 12th month. In this period there will be a natural thickening of the scar, as well as a change in color. This period is the least favorable stage of healing. As for hastening the natural process of healing, we recommend the patient waits, because this occurs in the final period.
LATE PERIOD: From the 12th to 18th month. During this period, the scar will become clearer and less consistent reaching its final appearance. Any assessment of the final outcome of the surgery should be made after this period. In rare cases it may take beyond this period to achieve the final maturation of the scar.

Some patients may experience unsightly scarring (hypertrophic and keloid scarring). This possibility should be assessed during the initial consultation as well as their family history. Light-skinned people tend to develop less of this type of scar. There are several medical and surgical means of improving such unsightly scars, at the appropriate time, not to be confused, however, with the natural evolution of the middle period of healing. Any questions about your scar development should be clarified during post-operative returns when we have the opportunity to make an assessment of the state of the scar. Occasionally fluid accumulation may occur in the operated area that may need to be punctured or drained, depending on the liquefaction of fat or blood collection.

WANT TO FIX HYPERTROPHIC SCARS? There are several medical and surgical means of improving such unsightly scars, at the appropriate time. Not to be confused, however, the "middle period" of the normal healing (the 30th day until the 12th month) as a complication scar. Any questions about its development should be clarified with your doctor.

HOW LONG TO REACH THE FINAL RESULT? In previous answer were some considerations on the development of the scar. However, some new observations on the abdomen remain, with respect to its consistency, sensitivity, volume, etc. 1-In the first months, the abdomen shows a relative insensitivity, and is subject to periods of "swelling", which should resolve spontaneously. 2-In this phase, the abdomen may look "stretched" or "flat". After some time, and beginning certain exercises, you will gradually reach the final outcome. It should not be regarded as a definitive result until 12 to 18 months post-operation.

WILL I HAVE A NEW NAVEL? No. Your navel will be transplanted and, if necessary, remodeled. You must take into account that, surrounding the navel there is a scar that will have the same development as the scar below. Several techniques exist for reimplantation of the umbilicus. All of them are amenable to future surgical revisions, should they become necessary.

DOES DERMOLlPECTOMIA CORRECT THE EXCESS ABDOMINAL FAT IN THE STOMACH REGION? Not always, it depends on your type of trunk (thorax + abdomen). If the patient is short, it will be partially corrected, however with the long type, the result is more favorable. It also depends on the thickness of adipose tissue (fat thickness) that covers this area of the body.

WILL PREGNANCY CHANGE THE RESULT? Your gynecologist will best advise you on the possibility of another pregnancy. As the result can’t be predicted, it is advisable that the offspring has already set before the abdominoplasty.

WITH A DERMOLlPECTOMIA IS THERE A LOT OF POSTOPERATIVE ABDOMINAL PAIN? The dermolipectomias usually have no pain and its occurrence can be controlled by common analgesics.

IS IT A DANGEROUS OPERATION? Every medical act includes within it a certain risk, and plastic surgery as part of medicine is no exception. You can minimize the risk by adequately preparing each patient, but not eliminate it completely. The surgical risk is not very different from other plastic surgery, when there are no aggravating factors and the validity of the usual surgical care.

WHAT TYPE OF ANESTHESIA IS USED FOR THIS OPERATION? General or epidural anesthesia, even local anesthesia under sedation can be used in special cases.

HOW LONG IS THE SURGERY? As long as the case may require. However, the time of surgery should not be confused with length of stay of patients in the Surgical Center, for this permanence also involves the period of preparation for anesthesia and postoperative recovery.

HOW LONG DO I STAY IN HOSPITAL? Normally 24 hours.

ARE DRESSINGS USED? Yes special dressings, changed periodically by the medical staff.

WHEN ARE THE STICHES REMOVED? The removal of the stitches can start around the 8th day; can also be in the days that follow. Rarely is the full withdrawal more than two weeks after.

WHEN CAN I TAKE A FULL BATH? Usually the second or third day, or after removal of dressings, assisted by a trained member of the team.

WHAT IS THE PROGRESS OF POSTOPERATION? One should not forget that before the desired result can be achieved, several phases are common with this type of surgery. Thus the end result depends on the healing progress, the shape of the abdomen, as well as sensibility, consistency, etc. Time and your body will take care of little problems that arise. Any concern on your part must be communicated to the doctor who will provide you with any further clarification needed. A short period of emotional depression may occur in the first week due to some temporary results and usually comes from an anxiety to reach the final result as soon as possible. Remember that no result of abdominal surgery can be considered definitive until after at least 12 months.
